I had a rhinoplasty 3 years ago that left me wih this protruding tip, and I’m afraid to have a revision since there isn’t much else to work with here in terms of structure (The nose was originally very large). what if the revision looks worse?

Hello, great question to ask.
Yes, the tip can likely be improved. A protruding or over-projected tip after rhinoplasty is usually a structural issue, not lack of tissue. A conservative revision focused only on the tip, often using cartilage grafts for control and balance, can refine this without making the nose smaller or worse.
The key is choosing a surgeon experienced in revision rhinoplasty. When done carefully, a limited revision does not have to risk the rest of the nose.

Surgery shouldn't make the nose look worse, because it's up to the surgeon to *not* do the surgery if he feels that he doesn't have an excellent chance of making the nose look better. You posted a 3/4 view that doesn't really show completely how protruding the tip is; a direct profile would be better, and photos should show the entire face, not cropped around the nose. That way, we can see how the dimensions of the nose fit with the overall size and shape of the face and come up with a much more reliable assessment.
