Large double chin, and my neck slopes downward - What procedure would best help me fix my neck? (Photos)

I'm 21, 5'10", and 170 pounds; however, I have a large double chin, and my neck slopes downward to the point where it is very hard to distinguish between my neck and jaw. I've also already had Kybella and Sculptra, with little to no effect.

Thank you for your question and for sharing your photographs. Based on my assessment, the fullness in the neck appears to be predominantly due to subplatysma volume. This type of concern is best addressed with a deep contouring neck lift, which allows direct treatment of the deeper structures responsible for the fullness.

In a younger patient, effective correction does not usually require skin excision. As a result, the procedure can be performed through a small access incision, approximately one inch in length, positioned discreetly beneath the chin, with no other visible scars.

There is also a degree of underdevelopment of the lower jaw, which contributes to the appearance of the neck and jawline. For this reason, neck contouring can be combined with a chin implant. This combination can significantly enhance the overall outcome by improving jawline definition and increasing chin projection, leading to a more balanced and harmonious profile.

You are 21 and not overweight.
Interestingly Kybella can cause fat hypertrophy in some people, making the problem worse.
From the photos you appear to have extra fat in the neck. Treatment will depend on if that fat is above or below the platysma (the thin muscle that runs from your collarbone to your jaw). If you grimace and feel excess tissue above the muscle then liposuction would benefit you. I would start with that at your age. 
If you are not in to exercise I would start by walking daily and modifying our caloric intake to nutrient rich foods and markedly limiting processed (fast foods, sugar rich foods) to decrease body fat.

Of course a facelift would help with this area but you are very young to go down that road.
