I recently went in for a consultation for otoplasty, the surgeon explained that he would be difficult to create the fold since i have a rare deformity. As you can see, my helical rim indents inward at the top. Sorry if i’m not using the correct terms

I understand that you are not happy with your daughter's results, and digital photos rarely tell the whole story. Please consider re-asking your question with both before and after photos. Though her right ear may stick out a bit more than her left, this may not be worth doing additional surgery...
If your ears protrude too far on front view, it may be because you were born with poorly formed anti-helical folds. Here to have a Stahl ear deformity. I hope this helps! Sincerely, Dr Joseph
I can see that there is slight asymmetry at the top of your ears on both sides, however, I am not aware of any surgical techniques to allow somebody's ear to be positioned further away from their head. And the up close photos of your ears do not have any Stigmata of surgery. Hope this helps!...
