When implants are placed behind the muscle, they can only go so far medially otherwise they can cut the muscle and will no longer be under the muscle. It appears your surgeon did the best he could with your anatomy and have a very good result. If you want more medial cleavage and upper pole fullness then I recommend fat transfer, if you have a suitable donor site. Larger implants will not make the situation any better and may in fact make it worse.

Thank you for your question. You are lucky that you have attractive breast as they are. If you want more cleavage, then I would perform a internal bra/capsulorrhaphy procedure. I wrote the very first paper on how to do this in 1999. I have been in practice for over 30 years and revisionary breast surgery is a large part of my practice. Generally speaking, there are three variables that determine breast augmentation outcomes.
The first is the patient’s candidacy for the procedure.
This includes variables like breast size, breast shape, breast position on the chest wall, breast divergence, etc., etc.,The second variable is implant selection in regards to size shape and type.
The third variable is the surgeon ability to put the implant in the correct anatomic location.
In your case, your candidacy for breast augmentation was limited based on having a small diameter breast that’s it far apart on your chest wall.
If the surgeon places the implants centered on your nipple, then the implants need to sit far apart because that’s where your breast are.
The surgeon cannot move the breast position on the chest wall.
If the surgeon places the implants more towards the midline to create cleavage, then the implants will not be centered on your nipples, and the nipples will be off to the side to where the implant is.
Patients who have small breast and choose larger implants will have a more implant shaped looking breast.
This becomes more obvious with implants sitting far apart.
Your breast have mild divergence on the left and minimal divergence on the right. The the divergent is fairly mild, but it does show slightly on the left side, and this can become more obvious with the projection of the implant.
While it’s possible to move the implants more towards the midline, doing so will leave the implants not centered in relationship to your nipple.
Overall, your augmentation outcome is fairly consistent with your candidacy for the procedure.

The bigger the implants, the more obvious it is that the breast sit far apart on the chest wall.
Overall, I think your augmentation was done correctly, and your outcome is simply based on your anatomic variation.
