I have fat and sagging skin at the jaw corners (below and next to ears), not the jowls. The fat is sagging below the jaw bone, on top of it, and above it. Many aging people present with that exact problem and their faces appear heavy, fat, and saggy because of that. The facelifts do not remove fat there. Can Ulthera be used there to melt the fat and contract the skin so it clings to the jaw bone?

Answer: Ultherapy is used to tighten and lift skin, but is not expected to reduce the fat at the angle of the jawline As Ultherapy can tighten and lift the skin it may help lift the skin and fat together raising it over the jawline from where it had sagged from the lower cheek. Despite some claims on the internet, the Ultherapy company doesn't claim that it can reduce fat. When I intend to reduce fat on a facial or neck area, I typically use either ThermiTight or Liposuction or both. Kybella is used on the submental neck below the chin. ThermiTight heats the fat cells to the point that is expected to cause the death of the cell which is much higher than the temperature that Ultherapy can cause. The information provided in Dr. Shelton's answer is for educational purposes only and is not intended to constitute medical advice. Answer: Ultherapy works for skin tightening, not fat reduction I would need to assess you in person to know for sure what type of correction will help you best. If the problem is restricted to skin laxity, a skin tightening treatment like Ultherapy might be a good choice for you. This treatment delivers ultrasound energy into the layers of skin to constrict tissue and boost collagen levels for firmer, smoother skin. Ulthera, however does not “melt” fat nor affect it at all, so it is not the right choice for you if fat is also a concern. If you are also seeking some facial contouring due to an enlarged jawline, you have a couple of options. If an enlarged masseter muscle is the culprit, Botox injections can reduce the muscle and slim the jawline. I could also perform liposuction in the area if the issue is isolated, stubborn fat deposits. Liposuction by the tumescent technique is usually what I do in this situation. No technology can precisely remove fat in that area, but a liposuction cannula can target the area right under the jawbone and above it. Often it is best to have the fat directly over the bone for maximum definition, but that decision is best made at the time of the consultation. I advise you to schedule a consultation with an experienced, board-certified dermatologist. Through a detailed assessment and discussion of your needs and goals, you and your doctor can design a treatment plan that meets your expectations and achieves your desired results.
