Sculptra is one option for volumizing the midface, but results can take many months to develop. You can consider other types of fillers, such as Juvederm Ultra Plus or Restylane Defyne, if you want more immediate improvement. Facial fat grafting is another option if you want improvement that lasts more than a few months. There is no substitute for an in-person physical examination that allows for palpation and manipulation of the tissues. However, from the photo provided, it does appear that many of your issues can be easily and quickly addressed via the use of injectable volumizing fillers. For example, your jawline jowls and lack of definition (as well as other regions of the face) would profit from the use of injectable volumizing fillers to elevate the cheeks, enhance the posterior angle of the jaw and restore appropriate shape and projection to the chin--three critical areas for framing the face. Addressing the apparent loss in cheek volume will help to lift and support that region and secondarily improve the lower face and jawline. Likewise, restoring volume to the posterior angle of the jaw will help to straighten a sagging jawline. And finally, enhancing chin projection will likewise soften jowl formation by pulling the anterior region forward, as in youth. In my NY office, I would opt for Voluma XC for the cheeks and Restylane Lyft for the posterior jawline and chin. In my Israel facility, where a far greater number of regulatory agency approved injectable volumizers are available, I would opt for Stylage XXL for the cheeks, Restylane Volyme for the posterior jawline and Neauvia Intense Flux for the chin. I stopped injecting Sculptra several years ago, since it does not provide immediate "Wow!" results as do the injectables noted above; it requires a series of about three treatment sessions; and is relatively expensive. Additionally, simple fillers, like ordinary Restylane and Juvederm typically used for fine lines and wrinkles will not do the trick for regions requiring volume and if used will not last long. So, be cautious when these cut-rate products are suggested for the locations where you appear to need them and make sure that you consult and receive treatment by a board certified aesthetic physician. . Excellent results in facial rejuvenation that look normal and natural are not gotten with filler alone. You would benefit from a face and neck lift, as well as fat grafting to perioral, periorbital, and midface regions.
Sculptra would be a very poor substitute for surgery, based on your photos; Sculptra would be a very effective volumizing adjunct to a facelift for the concerns that you raise. I love Sculptra as a treatment, have been a trainer for Scupltra in the past, but I will tell you that injectable agents have their strengths and weaknesses; Sculptra is most appropriate and works best in the upper/midfacial areas. The majority of patients will be underwhelmed by fillers/volumizers as a substitute for a facelift in this circumstance.

Sculptra is a great product for improving facial volume and collagen building, but for best results, I recommend using it in a combination depending on desired results. I combine Sculptra with other fillers, lasers, toxins and skin care to give immediate results.
Your photos show some sagging of the jowls and neck and it also appears that there is some volume loss in the cheeks. Sculptra can help to replace the volume loss and this can sometimes improve sagging jowls and marionette lines. I think a facelift to address the sagging of the lower face and neck would give more dramatic improvement though. Doing the surgery and Sculptra in combination would likely give the best results.

What happens during the facial aging process is that our fat pads migrate downwards and start to separate. We also experience bone loss. When we start seeing shadowing in the lower face, many times it can also be improved by restoring volume in the upper face, where the original volume loss occurred. You may also benefit from having dermal filler treatments in your jawline and nasolabial folds. There are a wide range of different types of fillers that have structural properties to restore volume where bone loss has occurred. Sculptra is a biostimulator that increases collagen stimulation. It can help with volume replacement over a series of treatments, but it does take several months to see results. While surgical facelifts can be very effective, they do not restore volume loss.
