I'm 3 and a half months past a fat transfer surgery to the face, unfortunately it seems that it was botched. There a 2 areas that are unnaturally overfilled, giving my cheek an uneven appearance and whole face asymmetrical. They feel a little hard to the touch also. Is there anything I can do to get rid of this fat? Whether massage at home or some surgery? Also will removal lead to further side effects?
Answer: Still too early Sorry to hear about your dissatisfaction with the results but it is still early and the fat/scar/swelling may not be completely healed at this stage. Topical scar massage may help. As stated there are many options ranging from steroid injections, 5-FU, dexycholic acid (off-label) to more invasive options such as radiofrequency and/or liposuction or surgical excision. Try to give it time. Rushing in to early can also create problems.

Answer: Facial Fat Transfer Without photos it is difficult to understand the degree of the asymmetry and/or overfill that you are concerned about. However, you are still relatively early in the post-operative phase. Procedures to the face can have swelling that lasts for some time- perhaps as long as 6-12 months in some cases. Therefore it may be advisable for patients to wait for a longer period of time to let the swelling settle. In addition, fat transfer is a complex procedure in that no one knows for sure the best way to do it- different surgeons feel more comfortable with certain techniques, which may require a lot of finesse in order to get the best fat survival. Therefore, the results in terms of getting the fat to survive may be unpredictable, even on different sides of the body in the same patient. Seeing your surgeon for an in person evaluation will allow you to discuss the treatment options. These may include massage, observation for a few more months, or perhaps even liposuction to help even out the areas. Doing liposuction to the face is not common, however, and may lead to even more contour irregularities, bruising, swelling, or pain. Or, you may need to have repeat fat injections to even out the areas. As always, it is best to seek out a well trained, board certified or board eligible plastic surgeon to do these procedures.

You may have swelling, which can last 3-6 months after fat transfer. Prednisone, icing, ibuprofen and venus legacy treatments can give improvement. I recommend prp and sculptra after your procedure to improve final results. If you're unhappy with your final results, RF treatments and 5fu injections can give improvement. See an expert. Patients often have pre-existing facial asymmetry that they are not even aware of. What you may be seeing could be some pre-existing asymmetry. Often times patient's aren't even aware of these asymmetries until after surgery when they are more focused on facial details. Massaging the areas may help. Massage helps break up scar tissue and helps remove some lymphedema or swelling. Arnica cream may help too. Remember you are still healing. Most surgeries are not completely healed for at least 6 months to one year later. Trying injections of a dilute Kenalog or 5-FU may help. May be worth a try if conservative measures and time haven't resolved things. Aspirating excess fat with microcannulas is also an option, but in my hands, it is the last resort. Good luck, hope things improve for you shortly.

Not many surgeons are comfortable in performing this aspect of reversing fat transfer. When I perform this aspect of reversing fat transfer, I usually do Micro Liposuction to the different areas that are overfilled. In some cases, I utilize a facelift type incision to visualize the injected fat that needs to be removed. This approach is utilized in more sever cases. With either procedure, the fat can be removed successfully. After all the swelling goes down, then you can proceed. Also, the injecting steroids or 5 FU usually is not successful. These types of solutions can also cause significant scarring long term and are usually not recommended. I hope this helps. Good Luck.
