Morpheus 8 is a non-invasive skin and soft tissue tightening treatment that is consistently amazing in our patients. We use the Morpheus to exact fractional induced RF into the skin or tissue in order to tighten collagen and stimulate contraction of the tissue. For acne scars, it is quite useful to build collagen in patients who have atrophic scars but is not very productive for ice-pick scars which require TCA Cross. For tissue tightening, Morpheus is ideal for the face and neck and we often combine it with CO2 laser or Ultherapy to further enhance results. Chemical peels and skin resurfacing should be done at least 5 to 7 days after, but can be performed earlier, depending on treatment severity. Microneedling can help reduce pore size because it causes controlled damage to the skin which will help it heal more correctly and produce newer cells. Everyone produces oil differently on their skin. Based on the picture alone, I see you do have a lot of oil production in your nose. I can also see clogged pores. To help reduce the appearance of enlarged pores and help slow down the pores from getting larger, I recommend you get on a series of Microneedling as well as a Hydrafacial MD, interchanging treatments. HydraFacial's help to unclog and clean out the pores. With microneedling the pores have an easy time closing with out having trapped oil and debris and can tighten the skin on area of treatment. Microneedling, also called collagen induction therapy, is a minimally invasive skin-rejuvenation procedure that involves the use of a device that contains fine needles. The needles are used to puncture the skin to create a controlled skin injury. Each puncture creates a channel that triggers the body to fill these microscopic wounds by producing new collagen and elastin. Through this process there is improvement in skin texture and firmness, as well as reduction in scars, pore size, and stretch marks.
I would recommend a series of microneedling treatments for the enlarged pores. I would also continue using a retinoid like retinol, Retin-A or Adapalene for the pores.
Thank you for your question and posting a picture. No one has perfect skin, but you can probably get significant improvement in your skin texture. If you take the surgical route, you will be trading one scar for another. You will get better and faster results with laser treatments over microneedling. After a complete consultation in a medical setting, you would probably be a candidate for a several treatment options based on the severity of the scar and how much downtime you can take. Keep in mind that more intense/deeper treatments require more downtime, but you will need less treatments. If you choose a short downtime treatment plan, you will need more treatments in the series. The most gentle would be something like the PicoSure FOCUS treatment with almost no downtime. It is safe for all skin types, but you would probably need 3 to 6 treatments. Another option would be a fractional 1540 nm laser treatment like ICON or Fraxel. With these treatment you could have redness and swelling for 1 to 3 days, but need less treatments. Look for a medical setting with a doctor in house, where they have multiple laser options. I hope this helps.

Yes microneedling can make a marginal difference however given your pore sizing there maybe an element of early phyma. Examination needed. If this was the case, lasers such as erbium or Co2 will yield a better result than needling.Microneedling has come a long way over the past few years and can help skin via several methods. Firstly it releases cytokines and growth factors in the epidermis and dermis- the top and bottom layers of your skin. This in turn remodels scars and the dermal layer of the skin. Additionally needles can break up scars themselves. This leads to new collagen formation, and in practice can provide limited skin rejuvenation. Microneedling is both user dependent and method driven. Simple needles can be used, growth factors can be added after the procedure, including PRP, or controlled heating during the procedure. This controlled heating is delivered via RF, and is called RF microneedling. RF can then be delivered via insulated or insulated needles. Even the energy level, depths, density, pulse duration and so many other variables can be controlled. This together with the added benefits of growth factor infusion makes this one of the most complex procedures in procedural dermatology. I use everything from Dermapen, though to newer generation insulated and non-insulated devices including INFINI micro-needling radiofrequency, Intensif and others. Newer generation devices can provide an added advantage in some, but not all patients. In my practice, I combine microneedling with many different forms of energy devices, depending on the expectations of the patient. Tailored microneedling combination will always give the best results.
