How to improve alar base reduction scars? 1 year post-op. (Photo)

Question

I had a rhinoplasty done almost one year ago with an alar base reduction, and so far im worried about the two scars on my nostrils. Is there a way to fix these or are these just permanent? The right side nostril scar looks less obvious compared to the one one the left.

Patients are always encouraged to speak with their surgeon regarding any questions they have about their follow up care. My patients are instructed to use a silicone gel to assist in the healing of their scars. Once the incisions have healed and the redness has subsided, generally at the 1 month post operative mark, they can start massaging the silicone gel into the scars. I have found that this helps with reducing the scar tissue as well as the discolouration. Scarring is the body's way of healing and is very different from patient to patient. Talk to your surgeon about options that are best for you.

Thanks for the question.  The reason why your scars are more visible is that there is a discrepancy in size between the ala and the remnant of the base that is was sutured to.  The ala is wider than the base.  This can be corrected with a scar revision and mattress sutures to maintain the correction.  This can easily be done under local anesthesia.

Thank you for the question and photo.  Revision of the scar would be the best option since it appears that the nostril edge is lined up a little high.  This would essentially involve opening the incision and suturing it so that the edges match up.  If there are any other issues with the appearance of the alar base reduction, that can be addressed at the same time.All the best,Dr. Remus Repta
