I really cant comment on the changes that are present today from your prior surgery, because there are no pre-operative photos. However your nose today has some deformities that can be further refined. The tip appears to be moderately bulbous and bifid, this can be significantly improved by carrying out a closed procedure, using sutures and appropriate cartilage tip grafting. It also appears that you have a partial "hanging columella" this could also be corrected at the same time in order to give a more natural and more feminine appearance.

Obviously the breathing issue can only be addressed with a physical exam. The tip has dome of lower lateral cartilages which are spread far apart. This is one of the few circumstances where I prefer an open rhinoplasty. Your tip should be totally rearranged and reconstructed to become more narrow and elegant, and fit your face better.All the best,Talmage Raine MD
