Although cosmetic procedures are never covered by insurance, certain procedures related to your surgery may be covered by certain insurance carriers. If your implants were placed for aesthetic reasons, it's unlikely that insurance will cover removal. If the insurance company considers removal of the implants medically necessary because of an infection or cancer, they may cover the procedure. I would recommend calling your carrier and asking if it is a covered benefit. Depending on your plan if it is covered they may require that you visit a surgeon in the network. If the implants you have were placed for cosmetic reasons, their removal is generally not covered. Your carrier will be able to tell you for sure.

Reconstructive explantation is usually covered by all insurances. Cosmetic explantations are only covered if complications of cosmetic surgery are not excluded. BCBS has some that are and some that are not, so check your exclusion page.
