Dr. Sexton performed my Tummy Tuck on 3/7/24. Dr. Sexton was very honest, straightforward and comforted me while in pre-op by telling me “I am gonna take good care of you”. You do not need a facelift, and you will pay a lot of money and see little or no difference. Your face is fine. Stay away from surgery for now and enjoy life. Greg Sexton, MD
I think it seems silly to do a breast augmentation and remove breast tissue at the same time. You’re taking away breast tissue and putting an implant back in to replace it. If you don’t have enough fullness or your breast is hanging, then you need a lift. If you don’t want to go up in size, you don’t want to breast augmentation, you want a breast lift. A lot of people don’t like the idea of a breast lift because of the scarring, but they will fade in overtime, but never go away. Again, if you want a better shape without changing the size, a breast lift is your main option. Taking out breast tissue and replacement with an implant will not change the fact that you have too much loose skin which is causing the problem. Good luck, Greg Sexton, MD.
Your pictures suggest significant stretching of the muscle wall. Liposuction will not help this, and in fact may make it worse. Impossible to tell without a physical exam, but I suspect you have very little fat between the skin and the muscle wall, which is addressed with liposuction. The bulging you have needs to be addressed by muscle repair, and again, I don’t think liposuction will help it. At best liposuction will be a waste of money, and at worse make things look worse than they are now. I would recommend a tummy tuck just by looking at your pictures, without an exam. looking at your breast, judging from the photos, you appear to have loose skin, and the distance from your nipple to your fold under the breast seems too long. Most of your pictures were taken with your arms up, which will make it look that way, but again it appears that distance too long. I would recommend repeating the lift. That would exercise the scars, and hopefully improve them, and at the same time, it could reduce the distance from the nipple to your fold, making your breast tighter, perkier and rounder. You would not have to touch the implant if you wanted to keep your implants. If you’re having problems with rippling, then I would go to a silicone, and possibly an extra cohesive silicone, which would give you more anterior projection and look perkier and rounder. When implants are under the muscle, we rarely see rippling at the cleavage line. You can get rippling under the implant or on the side where it’s not covered by muscle. Looking at your breasts it appears the pocket has been made too close to the midline, which puts the implant too close to the middle where it is not covered by muscle or breast tissue and that is probably why you’re seeing the ripples. You can tell it is too close to the mid line , seeing the nipples pointing out to the side. You really want your implant centered behind the nipple, not that far to the inside. Often the patient will ask for the implants to be closer and that’s when someone gets into trouble, if we put the implants too close, it makes the nipple point out and a good chance of getting ripples. I have never examined you, but I can think of two options that may, I emphasize may, fix the problem. Option one would be to take the implants out, repair the capsule, leave them out for three months, allow things to heal, and then go back and put new implants in the same pocket now covered by muscle and breast with more distance between the implants. Number two, take the implants out , stitch the muscle back down where it belongs and make a new pocket on top of the muscle, using mentors boost implants, which are extra cohesive and less likely to ripple. I think just trying to go in and repair the midline capsule, pulling it down and separating the implants without removing the implants or without making a new subglandular pocket will probably fail.
