I know there are surgeries for making asian eyes look caucasian, but is the reverse also possible? For example - making the eyes smaller overall and creating an epicanthic fold. For reference this is kind of what I'm going for

Patients wanting to change the anatomy of their facial structures often end up very unhappy with results. Even if this was possible, I would urge you not to seek such a surgery.

Answer: No there are no successful surgeries that make Western eyes look Asian.
Searching for this is a good way to make yourself a surgical cripple. The scary thing is that sometimes surgeons will delude themselves also regarding what they can realistically accomplish. You did not provide your own photograph. Sometimes what some one is actually looking for is a bit different than what they are asking for. In general the program you are describing in essentially impossible and likely to cause you harm and unhappiness.

Answer: Unusual eyelid surgery request
I am not familiar with any described procedure to take make an oriental style lid from a western style lid and I would question ones motive for this odd request. Just because something is possible does not make it logical.

Answer: Changing Western into Oriental Upper Lids
The muscle responsible for lifting the upper lid allowing us to look above the horizon attaches to the tarsal plate, a cartilage stave stiffening the margin of the lid, and tranversely to the skin of the upper lid creating the TARSAL FOLD which is accentuated and deepened when we look up and goes away when our eyes are closed. The point of muscle attachment to the upper lid is THE difference between western upper lids and Oriental upper lids. The Western upper lid has a higher attachment creasting a tarsal fold while the oriental has a low or missing attachment allowing the upper lid fat to cover its whole surface.
Creating a Western look involves creating a scar attachment between the muscle tendon and the skin at a preset distance from the eyelashes. There are various ways of doing so and it is a VERY popular procedure in affluent Asian countries (such as Taiwan, South Korea, Singapore and even Communist China.
However, removing the attachment to create a fuller Asian appearing upper lids is very hard because every procedure result is scarring. Removal of the attachments result in scar in the same location worsening the appearance.
I would suggest you stay away from such procedures.

If one is seeking a trans-ethnic look, we have to consider a craniofacial reconstruction as well as risking the result that may not "fit" your other features.
