Age 69, which type of revision is needed on my neck?

Unfortunately, your photos show persistent lower jowl bulges, along with fullness and looseness on the sides of your neck. The central depression under the chin may be where your surgeon wants to do fat transfer. That may help but I suggest a "redo" of the lower cheek and necklift with fat removal in the lower jowls along the jawline and along the sides of your neck, suture tightening of your platysma muscles posteriorly (toward the back), as well as further skin tightening. This may seem like a lot for just 3 months after surgery, but I think your photos indicate the need for much more tightening and careful fat removal in a few areas.

Most likely that you would benefit from redoing neck (fat removal, muscle adjustments) and lower face. It does not look that fat transfer will improve this situation without above mentioned revisions. Second opinion consultation with a board certified plastic surgeon, expert in face/neck lifts, is recommended. Good luck.

There appears to be excess skin in your neck still present, so you may best be served with a revision neck lift/facelift to improve the submental contour.

Secondary facelifts are generally very successful as there is less tissue to remove and the scar plane from the first procedure can be used to achieve a greater degree of tightening of the lower face and neck. Secondary lifts can be fairly common in patients with heavy necks. I would recommend you consult with another surgeon highly experienced in secondary lifts of the lower face and neck to learn more about this procedure.
