Neurotoxins for the reduction of submandibular glands

HI! I recently read in a scholarly article regarding the injection of certain neurotoxins into the submandibular glands in an attempt to reduce their size. The article suggests this method during a facelift or necklift as opposed to removal or a resection. Use of neurotoxin has been applied to treatment for sialorrhea in benign salivary hypertrophy. It works by reducing salivary production, thereby reducing size. I think some issues need to be known to patients before proceeding with this. First this treatment is early in acceptance and not every Med spa will be doing this. The treatment of skin wrinkles with neurotoxin carries very little long term risk. Salivary gland enlargement has multiple causes including malignancy which should receive medical evaluation and possibly imaging studies. If used on everyone without a work up as to cause, it’s possible that treatment could camouflage diagnosis until the problem has worsened. Physicians could probably do the injections and carry the responsibility although workup of these conditions is usually outside the specialty of many plastic surgeons.

Second, plastic surgeons doing facelifts look for long term surgical solutions. Previous studies indicate the treatment may need to be repeated in 5-12 months. This is not surprising and I don’t think of this as a long lasting solution as the article suggests. Because I believe in semi permanent solutions, I wouldn’t be doing this.
