Thanks for your question. I would keep it more simple. What bothers you the most? Do you want to be more full or do you think the nipples are too low? My opinion in analyzing the single photo that you provided is that a modest sized breast implant that fits your tissues based on careful measurements would "fill out" the breast and give you added fullness to the breast and especially in the upper poles. I routinely perform "dual plane" release with subpectoral (under the muscle) breast augmentation using silicone gel breast implants. The dual plane release can help "elevate" the nipples to some degree (and they don't need much). I would doubt that you would need a lift at all; however, after augmentation if you find the nipple it too low to your liking, then you can always consult with your plastic surgeon about adding a breast lift at that time. It is very difficult to give you good advice based on the single photograph; you may wish to post additional photographs, preferably with your arms by your side (frontal and side views) for more specific evaluation/advice. Otherwise, your best option is to see consultation with board-certified plastic surgeons who can demonstrate loss of examples achieving the types of outcomes you would be pleased with. Very important will be: careful communication of your personal goals; this communication is key when it comes to determining what operation would be indicated. Best wishes.
There are several options you have. I think you need a lift with medium sized implants. The vertical lift, also refered to as the lollipop lift will give you a nice shape with scars that heal very well.

Aloha, Yes, 'mild hiking' should be safe 3-4 weeks after breast augmentation? I usually allow patients to start light exercise ~2 weeks after, so as long as you're not rock climbing or doing the 'Stairway to Heaven', you should be ok. Best to see your Dr before you go, just to be sure.Mahalo, Dr D
