I went and got my lips done for the second time (with a different doctor each time) my injector started injecting a total of 1cc of juvederm but then she stopped and said: "I'm now gonna change to a different needle to give definition to your lips". How common is this? I didn't know you could remove or change to needles on the same injection? I just want to know is this safe and sanitary???

Swelling is not uncommon. Injections may cause lumpiness that can be either palpable or visible. Following up with your doctor and discussing your concerns with them is the best course. Results can be reversed with hyaluronidase.
Thank you for this question. Dermal fillers and neuromodulators create more subtle changes in the face and not the dramatic changes surgery creates. The problem with surgery is that "dramatic" many times looks overdone or "surgical". I don't think you can asses your results without looking at ...

Most injectors will use different types of fillers for the nose and lip
For the nose, projection is important so a filler that has high projection force and cohesivity will be chosen aka something "harder"
For the lips, which is a very dynamic area, a filler that is "softer" is usually chosen
