Answer: Lipo Candidate - VASER HD, J Plasma/Renuvion/Inmode BodyTite tightening, fat grafting for buttock lift/BBL, Cellfina/Sculptra liposuction alone or with heating will not be enough for you. you will need skin removal and weight loss/hormonal therapy along with exercise and dieting (lifestyle modification). You need a formal evaluation by an expert in body shaping and contouring to determine what is best. Traditional liposuction awake and smart liposuction are old methods and do very poorly for tightening skin or significant skin tightening or contouring. Small problem areas can be improved slightly by these methods. The best in body contouring not only removes fat with energy devices like VASER ultrasound but also internally heat the skin with radio-frequency devices such as J PLASMA/RENUVION, InMode BodyTite, ThermiRF to heat the under surface of the skin and tighten it at the same time as fat removal. These combinations are called 4D HIDEF VASER contouring and can only be done by a well . trained body contouring specialist. At the same time fat should be harvested, PRP and stem cells added and injected into muscles or indentations/cellulite to get body shaping and contouring. This is known as a Brazilian butt lift when you inject the fat to the buttock for contouring and lifting, but fat can also be used in areas such as the chest/breast, arms/shoulders, calves and face. Cellulite should also be treated with cellfina, sculptra, subcision and/or fillers such as bellafill, renuva synthetic fat, or sculptra. If skin is very loose before surgery then it will need to be removed such as lower abdominal skin (tummy tuck/abdominoplasty), breast skin (breast lift), arm skin (brachioplasty). Overall you need a full assessment, but simple liposuction alone will not tighten skin and needs a combination of various methods -- fat removal, skin tightening, defining, fat grafting, skin removal -- to get the best in body contouring and body sculpting. Answer: VASER Liposuction After looking at your pictures you appear to be a good candidate for gynecomastia surgery, possibly a tummy tuck, and high definition VASER liposuction of the back and flanks. From your comments it sounds like you want to take care of the excess fat and skin persisting around the mid section, back, and breasts on your body. All these procedures would take care of those problems and they can be done at the same time. A Skype or FaceTime consultation with a VASER specialist will help you get an accurate recommendation and pricing. Respectfully, Arian Mowlavi MD, FACS Realself 100 Hall of Fame Inductee

Answer: Number of procedures My recommendation would be to do liposuction of your entire torso as a single anatomic unit. This would include your chest, upper abdomen, lower abdomen, love handles, waist, back and sub axillary or armpit area. Based on your current size that would probably be at least 5 L and possibly more. Based on that you’re probably better off splitting the procedure. Men usually don’t have that much fat on Their arms but based on your size probably 500 mL of fat could be removed per arm. The chin doesn’t represent that much fat. Maybe 100 or 200 mL. The torso is the area where the bulk of volume is going to come out. If you want aggressive liposuction to the point of getting the best results it will probably need to go over 5 L of fat and most plastic surgeons will not do more than 5 L in a single session. Your primary problem is that you eat too much food. Until you get control over your eating habits liposuction may be somewhat limited in its ability to get you to where you want to be. Good old fashion weight loss by excepting that you can’t eat the amount of food that’s comfortable at all times should come first. Personally for me that included eating smaller portions throughout the day and simply excepting that I don’t have the luxury of just eating when I want to. The approach, outcome and results are going to be highly provider dependent. Some providers will only treat part of your torso and if that’s the case then you could probably have all the areas treated in one procedure but the results will not be as good. Find the right provider and everything falls into place including a proper plan of action. This includes the correct number of procedures.Adding Jay plasma is better than not adding J plasma but overall I haven’t been all that impressed with the outcomes from this device in regards to skin tightening. It seems to make the best or biggest difference for the chin and neck but my torso results adding this device has been only so-so. The best out, in regards to gynecomastia treatment will probably also require a separate open excisionof breast tissue that cannot be removed with liposuction. Best, Mats Hagstrom MD
