Should I choose implant size 270L & 300R filled to 280 & 300 OR size 300 & 335 filled to 320 & 340? I'm 5'4", 108 lbs, 34A. BWD 11.5 & 12. Desire full B to small C. My PS said my breast tissue is not too thin, it's pinchable. We agreed to fill the implant to the lower end of the range for softness & natural look. Please tell me which sizes you think will give me a natural result, & I can dress it down or wear push-up when needed? Will the 280/300 look too narrow on me?
Answer: Sizing of Ideal implant for a natural look. Both implant ranges would work for you. When breasts are different in size the determining factor is usually the smaller breast. As the width of the implant gets wider than the width of your breast it can start to look more like an implant, and when the implant width is less than the width of your breast it does not tend to look overly like an implant. The issue, of course, is that you need to use the larger implant in the smaller breast for the added volume. So the base width of the 335 ideal implants filled to 335 is 11.4 and this should still look natural with time in your 11.5 cm wide breast after the tissues have a chance to stretch. On another note, my patients with the ideal implant tell me it takes about 6 months after surgery for them to be soft enough to push them together in a pushup bra, so don't be worried if you can't do this at the 6-week mark. I agree with keeping the implants filled to the lower end of the range as the highest profile fill can give you more fullness in the upper breast in a "tight" breast than you might find desirable. It sounds like your plastic surgeon is sweating the details and sizing you carefully and this is the key to having a great result.

Answer: Size choices Go with the larger implant and stay in the middle fill range. This will allow you to get good cleavage haing a slightly wider implant. It won't be quite as round and will feel a bit softer by not filling to the max. Good luck.

Answer: Implant sizing Thanks for your question. I generally consider about 135-150cc to equal one cup size of augmentation (based on studies in the literature). So, if you are aiming for a moderate C cup, 280/300cc ought to fit perfectly for you! It will not be too narrow, I don't believe. Best wishes!

Given your desires and dimensions, either implant would be a very reasonable choice and either should result in a very natural appearing breast. Remember the amount of difference between the two implants is only 20 to 40 cc. When you consider that your present breasts are approximately 200 to 300 cc in volume, your total breast volume after the augmentation will be about 500 to 600 cc. Therefore, the difference between the two implant options is less than 8%. It will be very difficult to note a significant difference between the two implants you are considering. Don't loss sleep over this decision. I believe you will have a very nice result whichever implant you choose. Best wishes, Dr. Lepore.
