Hello cadillacThe incisions depend on the technique used by the plastic surgeon. I would a you schedule a consultation with a board certified plastic surgeon advice
The answer is no. In order to shorten the distance between the nose and the vermillion of the lip and lift the lip, excess skin needs to be removed. I do this by placing the scar in the crease on the side of the nostrils and in the nostrils as well as in the angle between the columella and the lip. In hundreds of cases I have not seen a significant scaring in the long term. Some scars can be red for a few months and have to be covered with some makeup for that period. However, long term they all heal well. Of course the surgery has to be done properly with an impeccable technique to get the best results.
No, a true lip lift would require an external scar. It would travel under the nostrils and tend to heal extremely well. Sometimes very large lips can be reduced in size via an incision within the mouth.Elliot W. Jacobs, MD, FACSNew York City

Thank you for your question. The technique I use for a lip lift involves removing the skin only. There is no need to remove any muscle since that may alter movement of the mouth after healing. The incision is hidden in the crease at the junction of the nose and lip, so it is not noticeable...
