What Id generally recommend in this circumstance is a breast reduction (which comes with a lift). It will give you a smaller and more symmetrical breast, with better nipple position. You will have a “natural” appearing breast with sloping at the top and more volume at the bottom.

You are a great candidate for either breast reduction or breast lift. They would both use the same scars, but if you have a lift, then all of your breast volume would remain. It would probably be a good idea to have a little bit removed to help with any pulling on your shoulders exacerbating your current pain. You can decide with your surgeon preoperatively how much to remove.
