In the photos it appears that your nipples are above your breast crease, and you do not have any droop. So I would only suggest abreast augmentation to achieve a fuller look. Good luck!

The breasts are a bit sagging, but not too much, so I think I can only enlarge them without lifting surgery. However, the current breast shape has the characteristics of a constricted breast and the gap between the breasts is wide, so it is better to consider a method of combining implant surgery and fat transplant for better results.

A side photo would help determine where the nipples are in relation to your breast fold. Based on the photos you’ve provided, however, your nipples may be located below your breast fold. This means that a breast lift would be recommended. Without one, the implants would sit too low on your chest and you wouldn’t have upper pole fullness.

I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. As others have said, an in-person examination would be needed to give the most well-informed recommendation. I have many patients wanting to avoid the scarring and cost of a breast lift and that is understandable. I do think that because of the asymmetry in the shape, sag, and areola in both breasts that a lift with implant would offer you the roundest, most symmetric, and most youthful results that will last you a long time potentially. You could likely still have increased cleavage and improved shape with augmentation alone, however I believe your best results would be from a lift.

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

Your photographs show that you have breast asymmetry (which is true for most woman). Your right breast is larger and more droopy than your left breast. You would get a better result with a lift but at the cost of scars and extra expense. If your expectations are for larger breast and more upper pole fullness, you can avoid a lift.

Due to a significant asymmetry of your breast it would be in your best interest to do breast lift and implant augmentation in the same surgery. It is right option for you and you should not compromise your good result to avoid scars. Hope this helps.

There is nothing wrong with wanting to avoid a lift. You can always begin with a first step of breast augmentation. You will need to choose moderately sized implants to fill out your breast envelope. If you decide you want a lift in the future, that is always an option. In my practice, I do not do augmentations and lifts at the same time.
