The breasts are a bit sagging, but not too much, so I think I can only enlarge them without lifting surgery. However, the current breast shape has the characteristics of a constricted breast and the gap between the breasts is wide, so it is better to consider a method of combining implant surgery and fat transplant for better results.

A side photo would help determine where the nipples are in relation to your breast fold. Based on the photos you’ve provided, however, your nipples may be located below your breast fold. This means that a breast lift would be recommended. Without one, the implants would sit too low on your chest and you wouldn’t have upper pole fullness.

An in person evaluation is really needed to determine if a lift is recommended for you. You do have asymmetry and one breast appears lower than the other. A breast lift can help improve asymmetry as well as correct nipple position. The augmentation will give you the volume enhancement and more upper pole fullness.
