Breast augmentation, i.e. fat grafting and lift is preferred. 32B post children/nursing. Suggestions? (photos)

With the limited fat you have available from donor sites and the lack of natural breast volume, I think you would be much happier with the outcome using implants rather than fat grafting.

Looking at the photos provided, the most accessible fat donor site would be the inner thighs and knees. I think this would provide enough for 1-2 cup increase. Otherwise I can't see any good donor sites. Thank you.

based on your photos, it looks like you don't have much extra fat so implants are probably a better option. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

The technique I recommend is a breast lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. At the same time or later, fat transfers can be performed if additional volume is desired.

You are thin and have little fat. Any fat placed will have a small impact on size and none on shape. A properly selected, highly cohesive silicone gel filled implant will give you the volume you want and the shape you want: natural. Breast augmentation doesn't have to look like balls. If you find it hard to find photos you like, it's because they are not the ones surgeons want to post; the world of social media only celebrates radical changes in plastic surgery.

It’s not clear whether you have enough donor fat for fat transfer. If you opt for fat transfer realize the increase in size will be minimal compared to implants

I would not recommend fat grafting in your case. From the photos you show, you have very little fat to harvest. Also, roughly 20-30% of the fat injected will be absorbed by your body. I feel like your results for AFT will be underwhelming and leave you wanting more.
I would favor a silicone implant in your case. They can still look quite natural if you stay under the muscle, modest size, and mod or mod+ profiles. In order to quote a size of implants you will need to be seen personally and your chest and breast measured.

For patients who are thin and want breast volume increase without implants ,I do internal lifting with absorbable mesh as well as advancing some of the tissue below the breast .

You do not have much donor fat so I suggest going with implants. Implants will give you a much more stable volume that will last many years.

Unfortunately, you do not appear to be a good candidate for Fat grafting. You have a very minimal amount of fat that could be used and it will make very little change for you. I would definitely recommend sub-muscular silicone implants. As far as size.... That is 100% up to you The best way to decide what size implant makes you happy is to be evaluated be a Plastic Surgeon who used Vectra 3D Imaging. I use this in my practice to show patients what they will look like with any size implant they want to see, until they decide what implant size will make them happy. It is over 99% accurate! The size you see is the size you will get! I have used Vectra for about 20+ years. Using this device, it is extremely rare for any patient to ever come back unhappy with their size, because they are the one who chose it. They are your breasts and it is your choice what size implant will make you happy. Good luck to you
