BBL with high blood pressure 135/91 - will my doctor have to cancel if it's that high on surgery day?

it is hard to tell for sure without an examination but you should be fine with that blood pressure. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

The first thing you should do is get your blood pressure checked on a different machine to make sure your device is accurately calibrated. You should definitely follow up with your primary care doctor. I suggest taking your blood pressure a couple of times during the day and write down each of the numbers since blood pressure fluctuates a bit from day today.
If you have hypertension, like lots of other people do then you should take medication until your blood pressure is well controlled.
Hypertension is called the silent killer, because it leads to reduced lifespan and serious medical complications, including heart, attack, stroke, kidney failure, and many other undesirable morbidities.
There are many different excellent medication’s to control your blood pressure and most of them don’t have significant side effects.
Even having mild hypertension has serious impact over a lifetime.
Each surgeon and anesthesiologist will have their own threshold when they cancel the surgery. The anesthesiologist may continue the case, but give you antihypertensive medication. This could slow down your case that day, and it would be a serious inconvenience to everyone involved. Surgery can be canceled because of hypertension, and each provider will have their own threshold.
Follow up with your primary care doctor, or any family medicine doctor, or internist to get yourself started on anti-hypertension medication if indicated.
