How to improve hollow under eyes? (Photo)

Question

41 yr old woman. Eyes look tired thanks to hollowness and shadows. Makeup products either don’t help or make the appearance worse. Not a good candidate for filler due to allergies (per dermatologist). What are my options? I’ve seen some great results with lower bleph surgery, plus fat transfer and CO2 combo, but not sure that makes sense if I don’t have bags.

Hi @moarsprinkles. I would not recommend a lower lid blepharoplasty for you. You would be a great candidate for tear trough filler, but due to your allergies I would also avoid tear trough filler. Another option would be nano fat grating to the tear troughs.

Thank your for our question. An in-person exam is necessary to make a definitive plan. However, based on your photos, you may be a good candidate for a transconjunctival blepharoplasty (to smooth the lower lid fat pads) and tear trough/cheek fat grafting to blend blend the lower lid contour and support the cheek. It does not look like you need skin tightening.

Hello, you actually are a good candidate for tear trough filler. This should be evaluated and performed by an oculoplastic surgeon, someone who is very very well versed in eyelid and orbit anatomy.

Since "you" state no HA fillers then try nano fat grafting to tear troughs. Expect fees in $1,500 ranges. Best to seek ONLY in person examinations.

What you have is easily treated with a "filler" by an expert. Your allergies should not be a problem since you can use either Juvederm Voluma, a hyaluronic acid filler with exceptionally low allergic potential, or Radiesse, a completely different filler (calcium hydroxyapatite), or fat with absolutely no allergic potential. You just need to see an expert, who the person you saw obviously is not. Under no circumstances consider a lower blepharoplasty or laser as you have none of the things that would qualify you for either.
