40M. I have hollowing under my eyes, which i feel is normal for my age. I had cheek fillers, whcih I was told would help the hollows (made them worse on the right side in my opinion) Wondering if a lower bleph would help me at all? I dotn thinknits necessary, but just seeing if anyone thinks it would eb beneficial

Hi there. You actually have a fairly common under-eye appearance for someone in their 40s, and I don’t think anything looks “severe” here. What I do notice is more of a hollowing and shadowing issue rather than significant puffiness or excess skin. In cases like yours, a lower blepharoplasty alone often does not dramatically improve the area unless there are obvious fat bags to reposition or remove. In fact, if too much volume is removed surgically in someone who is already somewhat hollow, it can occasionally make the under-eye area look more skeletonized or tired.
I can also understand why the cheek filler may have made things look worse to you on one side. Fillers placed too heavily or in the wrong plane can sometimes accentuate the transition between the cheek and lower eyelid rather than soften it. Personally, I’m not a huge fan of relying on fillers for this area long term, especially in thinner patients or men, because they can create puffiness, unevenness, or a slightly artificial look over time.
If you ever decided to treat this area, I think you’d likely benefit more from subtle volume restoration with conservative fat grafting rather than additional filler. Fat grafting can soften the hollow in a more natural way when done carefully. That said, based on these photos alone, I honestly don’t think surgery is necessary unless the under-eyes really bother you in everyday life. You still have a natural and masculine under-eye contour, and sometimes chasing complete smoothness around the eyes can lead to an overdone appearance that looks less natural than where you started. Your hollows are a volume problem, not a bag-excess problem. You have a negative vector. A standard lower bleph removes or repositions fat, which is the wrong tool when the issue is empty real estate along the orbital rim and tear trough.

The fillers you already have are doing some of the work, but if the right side looks worse, the product is probably sitting above the SOOF or stacking unevenly. If it's hyaluronic acid, my first move is to dissolve it with hyaluronidase and reset before adding more. Volumizing on top of poorly placed product compounds the problem.

For someone forty with hollowing only, the two reliable options are a careful tear trough and orbital rim refill with fat grafting to the lower lid, midface, and rim. Fat grafting is more durable and avoids the lumping that filler can create over time. Tear trough implants are also a good, predictable option. Hi there, thank you for your question and sharing your photos! You're asking if it may be time to under a lower blepharoplasty. At this point, I don't think you would see a significant benefit from a lower bleph, and in fact, the risk of complications would be greater than the mild improvement of a blepharoplasty procedure. You do not have significant protrusion of eye socket fat causing eye bags. You also do not have a significant amount of extra skin in the lower eyelid. I can appreciate some mild darkening of the lower lids and some fine wrinkles. This would be better treated with more superficial treatment using nanofat and a chemical peel or laser to resurface and restore the skin texture and pigmentation. A lower blepharoplasty should be done when you have more significant contour issues. Hope that helps!

For a 40-year-old male with under-eye hollows, the typical concern is volume loss creating shadow, not the kind of fat-herniation problem that classic lower blepharoplasty addresses. Going straight to a traditional bleph in this scenario often creates a worse appearance &mdash; a hollow lid that looks more aged, not less.
What usually works better:
Conservative tear-trough filler. HA filler placed deep on bone in the medial tear trough can fill the shadow directly. Reversible, lasts 12 to 18 months, low-risk in skilled hands. Good first move to 'preview' what fuller anatomy looks like before committing to surgery.
Autologous fat transfer. Your own fat grafted into the trough area. More permanent than filler (50 to 70 percent retention), looks natural because it's your tissue, but requires a small harvest from another area.
Lower blepharoplasty with fat repositioning. If you do have any fat herniation above the hollow (not always visible in selfies), this technique re-drapes that fat into the trough instead of removing it. Solves both the bag (if any) and the hollow in one move.
What I'd specifically avoid for you:
Traditional lower bleph with fat removal. Removes the fat pads that contribute volume &mdash; patient ends up more hollow, not less. Common male regret case.
Heavy filler that overcorrects the hollow into a puffy lid.
Best move at 40 is a consultation that includes both injectable and surgical experience. If a surgeon only does one or the other, you'll get steered toward what they perform.
Send clear photos in even lighting from front and 45-degree angles &mdash; selfies in dim light overstate the appearance of hollows.

While a lower blepharoplasty might seem like a "big" step, it is actually the most definitive way to address this because it allows a surgeon to perform a fat transposition&mdash;essentially taking the natural fat you already have under the eye and repositioning it to fill the hollows, creating a permanent, seamless transition. This often looks much more natural than filler because it uses your own tissue and won't ever look "puffy" or uneven. If you're tired of the "tired" look and fillers aren't hitting the mark, a consultation for a conservative lower bleph could be very beneficial to see if moving that tissue around is the missing piece for you.
