Hello! I was curious, is it safe to get a fat repositioning lower bleph to fill in tear trough lines? I saw a doctor on here talking about how repositioning fat in the lower lid can cause blood flow loss, tension, bulging, and ither unpredictable outcomes due to the way to fat is re sutured to the face. Can anyone give me insight on this?

It's one of the most reliable lower eyelid procedures we do.
The approach moves your own existing lower lid fat (which is herniating slightly and creating the bag above) downward to fill the tear trough below. It solves two problems with one operation, and because the fat is your own tissue, it lives long-term without the cyclical absorption you get with filler.
Safety profile is excellent in experienced hands. The dissection is conservative, no external incision is needed for the transconjunctival approach, and the recovery is typically 7 to 10 days for visible swelling. Complications are rare and usually mild. The main risks to know:
Lower lid retraction if too much skin or muscle is taken (but transconjunctival approach typically avoids this).
Asymmetry, since the two sides can heal differently and small revisions are sometimes needed at 6 to 12 months.
Overfilling the tear trough if too much fat is repositioned, which is hard to reverse since it's your tissue, not dissolvable filler.
The most important variable is surgeon experience. A surgeon doing 50 or more of these per year has refined judgment on how much fat to mobilize and where to anchor it. Less experienced surgeons sometimes overcorrect.
What I'd ask in consult: how many they do annually, what their revision rate is, and ask to see results in patients with your exact starting anatomy.
If your tear trough is mild and you want to test the appearance before committing to surgery, deep filler placed by an injector with extensive under-eye experience can preview the effect for 9 to 12 months. But for durable correction in someone with even minor fat herniation above the trough, the surgical approach gives the cleanest, longest-lasting result.

Hi there thank you for your question and sharing your photo! I can appreciate the more prominent tear trough you are experiencing. Have you been worked up for thyroid issues? This can sometimes cause puffiness in the lower eyelids. 
With regards to surgical intervention, fat repositioning is a commonly performed procedure. in younger patients, this often times is not as necessary, and this can be done with orbital fat removal without an external incision. Hope that helps!
