I think you can achieve a very nice aesthetic result with a revision surgery. The previous place where the incision was made cannot be changed, but the scar itself can be revised. I personally would treat your case like a tuberous breast surgery case. The constriction at the lower pole of the breast is either due to the scar from the previous surgery or the native structure of your chest or a combination of the two. If it is just scar tissue from the previous surgery, that can be cut out so that the lower pole of the breast can expand. After that, it would be a matter of trying to prevent the keloid scar from recurring. To do that, I would recommend using steroid injections at that site or 5-FU injections at that site, and aggressive scar massage once the skin has healed. If the constriction is due to the natural bands within your tissue (where your natural inframammary fold was located), that could likely be addressed with a technique called ‘radial scoring’ where the tight bands within the tissue are released. That plan doesn’t change the position of the scars, but it would hopefully make the scar thinner and less noticeable, and improve the shape of the lower pole of the breasts.
With respect to the overall shape of the breasts, it depends on what you are wanting to accomplish. To make the breasts project less outward, you could change the profile of the implant. Removing some of the breast tissue would be another way to decrease the projection of the breasts, but that would essentially negate the breast tissue growth that you achieved with your hormone treatment.
“Top surgery” is performed by a wider range of plastic surgeons, but there are some plastic surgeons who complete additional training and do a fellowship dedicated specifically to gender affirmation. Those surgeons also perform “bottom surgery,” but they generally have more experience performing “top surgery” than most other plastic surgeons. While most board-eligible or board-certified plastic surgeons are qualified to take on a case such as yours, I would recommend researching surgeons in your area who have completed a fellowship in gender affirmation surgery.
