1 year after augmentation – right breast firmer, painful, and saggy with no upper pole fullness. Is this normal? (Photos)

I’m 1 year post-op from breast augmentation and have some concerns about my right breast. Part 1 – Symptoms • One day while bending over, I felt the right implant “shift” and then fall back into place. • Since then, the right breast feels firmer/stiffer than the left with ongoing discomfort. • Pain and stiffness are worse when I’m lying on my back. • I have stinging, internal nipple pain on the right. • Lying down, I can feel a small, hard/lumpy area in the right breast. • Overall, I have no upper pole fullness and the breasts, especially the right, look more dropped/saggy compared to early after surgery. Part 2 – Questions 1. Based on these symptoms, does this sound like a normal change 1 year after augmentation, or more like a problem (such as capsular contracture or implant malposition)? 2. Is this level of drop/sag and lack of upper pole fullness typical at 1 year, or does it sound like the implant may be sitting too low and need revision? I understand you can’t diagnose me online and that I need an in-person exam, but I’d appreciate your thoughts. Thank you.
