Is Nose Straightening/Tip Reduction possible for AERD/Samter's Triad? (Photos)

Question

I'm a 23yo female, 5'3", 120lbs. I have AERD (nasal polyps, chronic sinus issues, asthma, aspirin allergy), for which I had a septoplasty (with turbinate reduction and polyp removal) in July 2020. Since this initial surgery, my nose has grown very crooked and my breathing is severely disturbed. I take Dupixent, which helps with the polyps, but I am hoping to get surgery to straighten out my nose and make it less bulbous. To the extent possible, I also want to get rid of my dorsal hump and bring my nose in. I realize this is a big ask so I want to make sure my expectations are realistic, would love any thoughts.

You are definitely not asking too much. Your expectations are completely reasonable, and if you decide to undergo rhinoplasty, those concerns should absolutely be addressed as part of the procedure.

We have seen people with your condition have recurrence of sign or nasal polyps, after surgical removal, as soon as 12 months postoperative. Others, however, end up getting years of benefit from their surgery. We can see that you have a crooked nasal appearance, a long nasal appearance, and a sizable dorsal hump. The best time to have considered rhinoplasty would have been at the time of your septoplasty, however, if you do not have adequate nasal septal cartilage remaining for nasal tip reconstruction and support, an MTF Profile sheet of cadaver cartilage could easily be purchased for reconstructive purposes. In our practice, we do offer nasal polypectomy and endoscopic sinus surgery, when it is necessary. You should consider Consulting with a local reputable rhinoplasty specialist who is also capable of dealing with your possibly persistent nasal polyps, so that you end up with a better looking nose that also breathes better after your surgery.
