I have high, shallow cheekbones and as I've aged, the "jowl" area of my face has become more prominent. I don't mind this so much, but I do not like the persistent lines in my nasolabial area (see picture). I get Botox for my frown lines and crows feet and love the results. Is there anything similar for this for nasolabial folds? Which treatment would get me closest to "eliminate wrinkles without adding too much volume"?
Answer: Best Options for Nasolabial Folds Hi and thank you for your question! There is really no better option than filler treatments for nasolabial folds. The issue you mentioned about adding too much volume is really an issue of the technique and experience of the person who does the injection. Less experienced injectors tend to focus only on nasolabial folds, while more experienced cosmetic doctors place their injections strategically in other areas such as cheeks to general a nice lift in nasolabial folds. I treat several patients for the same issues everyday and my patients hardly ever see issues with over filling. As long as you work with an experienced cosmetic doctor, you shouldn't have any issues with adding too much volume. I hope it helps and good luck!

Answer: Dermal Fillers Hi @Chicago5725! Thank you for sharing your photo--hyaluronic acid dermal fillers would be a good option for you. I recommend cheek fills. This lift would soften the nasolabial folds. Additional filler to the nasolabial folds can also help soften the area. Restylane Lyft and Juvederm Voluma work great in cheeks and Restylane Defyne works great for the nasolabial folds. Best of luck, Dr. Nima!

Answer: Best treatment for nasolabial folds with minimal projection Thank you for your question. Total facial rejuvenation begins with a complete history and physical examination coupled with a detailed understanding of an individual's expectations. The aging process is different for everyone, some people loose more volume in their face whereas others may gain it. By looking at a single photograph taken at one angle, not evaluating other angles may not be the best way to make concrete suggestions. Nasolabial folds are due to lack of mid face volume and hence adding filler into the nasolabial folds is not the way to correct them anymore. Factors that have been associated with accelerated volume loss include extreme athletic individuals, extreme restrictions on diet, severe sun exposure, and lifestyle habits. I strongly recommend an in-person consultation for an individualized treatment. Best Wishes, Dr. Shanthala

Answer: Facial volume loss Hi,Thank you for including a photograph. As we age, it is common to have deflation and descent of the facial fat. This problem can be compounded with rapid weight loss. In cases such as yours, the best options are to revolumize the areas most affected by the weight loss and in some instances we can use minimally invasive techniques to resuspend the sagging skin and fat. Your best and most straight forward option is to use a thicker hyaluronic acid (dermal) filler to revolumize the cheek. After this is done, you may not need anything done to the nasolabial fold area. If you want to be a little ore aggressive, you can combine the fillers with a lift using PLLA threads (Silhouette InstaLift) which are essentially and solid version Sculptra. They are bi-conal threats that give BOTH a lift and help your body make new collagen. It would serve you well to see a facial plastic surgeon to discuss all available options. All the best,Mehdi Sina, MD

Answer: Severe volume loss fixes Hello and thank you for your picture. One of the reasons for your nasolabial folds to become so prominent is the fact that your face got thinner from the rapid weight loss. Your cheeks have lost fat and support, which created the nasolabial folds. You can definitely try filling your NL lines first with dermal fillers like Juvederm Vollure or for best results, add fillers to cheek bone area to lift the skin and soften the NL folds. Dermal fillers are your best, quickest and safest option. Ultimately, I would suggest you consider Sculptra for more long term option and more for facial support rather than volume addition. Please see an expert in your area. Best, Dr.Avaliani
