Now that mass health covers lipodystrophy patients (FINALLY!!) I am looking forward to regaining mass in my backside and I am actively searching for a dr in the Boston area who can help me with my dilemma. Long term survivor here, and healthy other than this unfortunate side effect of the now outdated hiv drugs.

Your butt implants will not necessarily sag over time, but sciatica is a known complication. Implants can put pressure on the sciatic nerve, but usually when this happens it gets better with time.
