Butt Implant moved? Why is it so painful, there are no signs of rupture according to the MRI result? (photos)

Gluteal implants carry a relatively high risk profile.  Risks of infection, displacement, extrusion (rejection), nerve impingement and contour irregularities such as yours are real and probably more common than other plastic surgery procedures.  It is unclear what is causing your dimple to become worse or painful, but may simply be the traction that the implant is creating on the scar capsule that surrounds it.  Rupture of the soft, solid silicone buttock implants that are placed in this country is not a concern since they are solid, so I'm not sure what the MRI added to your evaluation other than to look for impingement on your sciatic nerve.  Fat transfer into the dimple and over the implant may help to correct the depression.  Fat transfer BBL after removal of gluteal implants is usually delayed for 6 months in my practice to allow the implant capsules to close down and avoid injecting fat into a non-vascularized space.  Good luck!

You are best to be seen in person to determine what the issue is. Is the implant displaced lower in the buttocks?

Since your implants are textured, a small rip in your muscle fascia can can cause the implants to tether up to the skin causing dimpling and pain; I would recommend that you leave the implants in place and utilize fat grafting over the dimpled region to release any tethering and fill in the dimples with fat grafting to smoothen out your results and add more volume especially onto your lateral cheek dimple.
