The nipple is not actually cut off the breast for a lift, but stays attached to the breast. Skin is opened around the nipple so the breast mound can be repositioned. I hope this helps.

I would suggest doing the implant or fat transfer first and restore the volume you lost, then see if a lift is needed. It might not be needed. Implants will probably be more successful than the fat transfer because of the scarring from the infection.

You should be able to have the doughnut lift with the areolar reduction and implants. Implants will give a better shape and more volume than fat.
