I have had "tired" looking eyes for my whole life, and I am looking for the best treatment/procedure to treat the area under my eyes. Thank you!

Hi,

Yours is a very common concern! The most noticeable conbributing factor is the formation of grooves calld tear troughs, which can be tricky to treat. Options include dermal fillers (temporary with variable treatment duration) versus under eye surgeries involving fat transposition from the cheek vs transfer from elsewhere. I recommend seeing a board certified surgeon in-person for further evaluation. Hope this helps!

Based on your photo, you are a good candidate for lower lid fat removal coupled with laser resurfacing of the skin and fat injection into the tear trough. This can result in a nice smoothing of the lower lid area which looks very natural. Be sure to see a facial specialist for consultation.

It seems you have concerns about the under-eye area.Common issues under the eyes include hollowness, wrinkles, and protruding fat, which often contribute to a tired appearance.You are an excellent candidate for lower blepharoplasty.Through an incision just below the lower lash line, the tear trough can be smoothed out, protruding fat can be redistributed to fill hollows, and sagging skin can be tightened to create a more youthful under-eye area.Typically, stitches under the eyes are removed on the 7th day after surgery, and the healing process significantly improves the appearance of the incision.

There are several nonsurgical treatments for "tired eyes" that are worth considering. A nonsurgical robust HA filler cheek augmentation can not only contour the cheeks, but secondarily smooth the transition between the upper cheeks and the lower lids, thereby diminishing bags and dark circles. 
A series of in-office superficial peels, using high concentrations of glycolic or salicylic acid, Jessner's Solution, or low concentrations of TCA coupled with the regular at-home, topical use of prescription strength retinoids, and alpha hydroxyacids can further improve the tone and textural appearance of the periorbital region. You would be wise to consult with a board certified cosmetic dermatologist who can help you with all these approaches. Hope this helps and best of luck. .

Lower blepharoplasty is good procedure to address deep circles, tear trough deformity. Transconjuctival  blepharoplasty where incision is placed inside the eyelid may be best option in your case; periodical fat grafting is also to be considered. Good luck.
