Do I need a Breast Lift? If so, what incision? (photos)

This is always difficult to answer without an in person exam to perform measurements and determine skin quality. I really do not prefer to do a lift on patients in your age range for multiple reasons. I will say a downward facing nipple is not corrected by an implant alone.

Your nipples are located below your breast fold and are pointing downward, therefore a lift is probably the most suitable option for you. This would raise your nipples as well as make them project more forward, rather than downward. If you were to only get breast augmentation, the implants would sit rather low on your chest as they would need to be centred behind your nipples. You would probably need an anchor incision, although that could be further discussed with your surgeon.

Dear Friendly987678, in order to achieve your desired goal you would need breast augmentation with dual implants, this would give you an amazing result, about the scarring we can take preventives measurements also manage the asymmetry. I recommend an in person evaluation with a plastic surgeon to be measured and discuss your goals. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants, if desired. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. Women tell me that this process is very helpful in determining what size and style breast implant is right for them. Mildly Sagging Breasts: If your breasts are mildly droopy, a saline or silicone gel implant placed behind the pectoral muscle might help create the illusion of perkiness. That’s because as the implant fills out the top of the breast, it also fills out the bottom, making it look as if the nipple has moved higher. It hasn’t. Your breasts are low on the chest wall, the nipple-areola complexes are pointing outward and you have poor cleavage. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers or small implants can be placed totally submuscular.

I would not perform a breast lift on somebody as young as you. It would require scars and an implant. Your breasts aren't that bad at all. Wait til after you have children.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

My recommendation for you is a breast lift with implants, if we only increase the size of the breast they’ll look more saggy not delivering good results. The type of incision in your case I’ll suggest a lollipop, so we can diminish the size and direction of the areola also lift them on the proper distance.
