Hello and thank you for your question. Based on your photographs, you may benefit from a revision to help improve the symmetry of the dorsal aesthetic lines and reduce the dorsal hump. Placement of spreader grafts during the revision will help accomplish this. With good surgical technique, you could have an outstanding result. From these photographs, you have a residual dorsal hump, wide nasal bones, and inverted upper lateral cartilages consistent with an inverted V deformity. A copy of the operative report would be helpful, and it's very important to know how much cartilage is left over inside your nose for grafting purposes, since you're gonna need bilateral spreader grafts to widen the upper lateral cartilages in the mid vault, that are collapsing, in addition to osteotomies placed in the nasal bones. All this can be done with a closed rhinoplasty approach with all the incisions placed on the inside of the nose, and no packing placed inside the nose. Revision rhinoplasty is very difficult, so choose your rhinoplasty specialist very wisely based on extensive experience
