As far as I can understand from your question in some cases, it affects the appearance, for this reason, we recommend lip lift operation and rhinoplasty operation together in patients we deem necessary in operations.

When we perform rhinoplasty, we almost always place a columella strut graft that extends from the tip down to the premaxillary bone. Since this cartilage graft is placed through the upper lip muscle, it may feel a bit heavy to some for several months after healing. As long as upper lip muscle is not excised or significantly traumatized, there should be no significant change in somebody's smile after rhinoplasty. That said, if somebody has a long nasal appearance and a droopy tip, after rhinoplasty, there may be more visibility of somebody's upper lip. I hope this helps! Sincerely, Dr Joseph

Routine rhinoplasty does not affect the lip. However, if the surgeon does decide to remove tissue from the nasal spine or soft tissue from that area then it can change the lateral profile.

In a situation like yours the nose is exerting tension on the upper lip. A rhinoplasty will often cause the upper lip to be less tight especially if the caudal septum is shortened as you mention

If there is "more" stuff at the nasal spine, which is the front base of the nose, then it will naturally put pressure down on the upper lip region. The "stuff" could be things like the septum cartilage itself and/or any additional cartilage put in that area like a septal extension graft. Consequently, removing cartilage from this area would thus "release" the upper lip pressure/strain.

Open rhinopalsty should not affect the function of the upper lip in any way. If you have already had a rhinoplasty, follow-up with your rhinoplasty surgeon so that he or she can address any of your concerns.
