It looks like you may benefit from a revision to achieve more upper pole fullness, as your nipples are located above your breast fold but the majority of your breast volume is still below.. An implant may also help.

Thank you for your question. There is still a lot of tissue remaining in the lower pole. A revisional mastopexy that removes more lower pole tissue and makes the areola more circular is needed. Sunken upper pole and central part will require fat grafting.

Hello @Cute221440, thank you for your question. Please remember that the the capsules must be removed as well to prevent a future inflammatory response. That said, you could benefit from a breast reconstruction with your own tissue. It is almost always painless, the scar is L-shaped. There are no scars in the cleavage area, and the aesthetic result is satisfactory, stable, and consistent. Due to the BRALAN (Breast Reconstruction Anatomical Lift Augmentation) Technique, the anatomical shape and projection of the neckline are independent of the volume. You would, very likely, benefit from lollipop type of lift to get your nipple on the top of the brest mound. In same cases fat grafting over upper pole and medial aspect of breasts may imrove shape and size. Good luck.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

One can argue whether a little more skin tightening is necessary (a revision lift), but what is certainly going to vastly improve your shape is a breast implant. Fat grafting can be considered, but it is less reliable and has less 'lifting power' to give upper pole fullness and projection of the breast mound directly underneath the nipple areolar complex. I think that a properly selected implant will give you everything you want without any need for further skin removal.

You’ve got a perfectly fine result. Unfortunately most breast lifts without an implant lack upper pole fullness. So you need to decide if you want modest size implants to give you that fullness.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

You do look better than your preop photos, but I think you could certainly look better. Your nipple to breast fold distance is too long making your entire breast volume be below your nipple. If this distance was shortened with a scar hidden in the breast fold, you would look much better. Adding volume above the nipple with fat grafting could also make it better. Ideal to me would to do both procedures at the same time. Discuss options with your plastic surgeon. Good luck.

Your skin appears to be fairly stretchy and unable to hold your breast well. Please don't choose implants for you will sag even more. The lift can be revised, especially from the inferior incisions, and retighten your breasts and go somewhat smaller.
