Bad Necklift for 3rd Revision? (photo)

A suture suspension neck lift is not a true neck lift.  While it may contour your neck, the underlying anatomy (muscles) are not tightened and the overlying lose skin is not removed.  Additionally, all of the dermal attachments causing wrinkles in the skin (as seen in your photographs) are not release, so you basically end up with a bunched up and unnatural appearance.  Suture suspension neck lift is good in some candidates, but if you are looking for improvements, I would suggest a traditional short scar neck lift so that the skin can be elevated, the dermal attachments release and the underlying muscles tightened.  This is the what will give you the best and most long lasting results.

I am sure this has been stressful for you. While difficult to comprehend, time is on your side. you will continue to normalize. Do not rush into any surgery. Get a few different opinions. Be sure your physician helps you understand what realistic expectations should be at this point.

    A revision may be reasonable, but I would have to see the pictures from different perspectives or examine you in person.  In addition, it would be helpful to see the changes in your face and neck from before the first surgery and the progression.  Although many surgeons will tell you that something can be improved it is important to examine what has been done and how your anatomy has responded.  Kenneth Hughes, MD Los Angeles, CA

Thank you for the question and photos.  I am sorry to hear about your experience.  It appears from your photos that you have too little volume ( fat) between skin and neck muscles.  I can visualize the outlines of the neck muscles through the skin.  You will definitely need to discuss your goals as well as the limitations with an experienced board certified plastic surgeon.  You may need some fat grafting to smooth out the contour of your neck.

You will need a revision to improve the neck contour. An examination and review of your previous surgical record will be necessary before deciding on the exact surgical approach.

You are wise to wait 9-12 months before considering a revision.  There are certainly some irregularities in the neck that could be addressed. Done skillfully by a Board Certified Facial Plastic Surgeon, a revision Neck lift is not an unreasonable request but the surgeon would need more information about the exact technique and incisions used in order to plan the revision procedure.
