Coming up on my 5 month post-op review - how should I speak honestly about my concerns without fear of upsetting my surgeon?

I’m due my 5 month review next month. I don’t know if I’m being overly critical but I’m still concerned that things aren’t as tight as they could be. I’m happy to talk to my surgeon about options, if they are needed, but I’m feeling awkward about the conversation and don’t what to come across as being ungrateful or it’s a reflection on his skills as a surgeon, I don’t want to offend him . Should I speak honestly without fear of upsetting my surgeon? I don’t like confrontation
