Non-cosmetic rhinoplasty - what condition is being described & surgery recommended?

I have problems breathing and a few months ago saw a doctor in Europe regarding my condition. In the medical report there are two interesting moments: "Distinct suction phenomenon of both nasal wings" in findings section and "two-stage surgery of the alar wings" in recommendations. Can anyone explain me what condition he could be describing and what surgery specifically recommending? All info I find online talks about cosmetic reasons for this surgery, not medical ones
