In my opinion, it’s not ideal to combine a full Mommy Makeover (tummy tuck, breast lift/augmentation, and 360 liposuction) with a BBL—even a “skinny” one—in the same operation. Here’s why:
Surgical time and safety: Combining all these procedures significantly lengthens operative time, which increases anesthesia and complication risks. A fatigued surgical team can also impact precision and outcomes.
Recovery conflict: After a Mommy Makeover, you’ll need to rest on your back; after a BBL, you can’t sit or lie directly on your buttocks. Balancing those restrictions makes recovery very uncomfortable and can compromise both results.
Best sequencing: Most patients get the best outcomes by staging—starting with the BBL (to shape and add volume to the hips and buttocks), then performing the Mommy Makeover 6–9 months later once healing and fat retention are stable.
As for your flat nipples, yes, they can often be improved during your breast lift. If your nipples project when cold or stimulated, that means your underlying anatomy is responsive, and your surgeon can restore projection by tightening and repositioning surrounding breast tissue during the lift but your underlying anatomy and responsive can't be improved surgically.

Since your nipples can respond, even if only occasionally, the lift usually makes them look perkier and more balanced.
As for combining a “skinny BBL” with a mommy makeover, yes, this can often be done at the same time. The key is being realistic about the amount of fat available for transfer. With your goal of just a subtle enhancement, this is usually very achievable in one surgery, provided you are a healthy candidate.

Thank you for your history and your pictures. The short answer is "yes." Regarding your abdomen, you would be best served with a full abdominoplasty, also known as a tummy tuck. This would include the removal of excess skin and fat from the abdomen, repositioning of the belly button, and lifting and reducing of the pubic region. It would also include liposuction. Most plastic surgeons would suggest liposuction of your waistline and possibly even fat transfer to the hips for a more defined shape. In addition, liposuction can be performed on the superior iliac crests (also known as "love handles") as well as the lower back. This is called 360 Lipo. You could also undergo bilateral axillary or armpit liposuction and chin (or, more accurately, neck) liposuction. These procedures can be combined simultaneously. You may also consider breast augmentation. With a breast lift, the size of the implant you choose will determine the type of breast lift (mastopexy) you need. You may need a donut mastopexy or a lollipop mastopexy. If you choose a relatively small breast implant size, you may need a full anchor mastopexy. To correct flat nipples, several options are available. Flat nipple correction procedures often involve fat transfer beneath the nipple. However, fat grafting to the nipple could decrease nipple sensation. Therefore, if nipple sensation is a significant concern, consider other options. Another very popular procedure to correct flat nipples includes the placement of a permanent suture around the perimeter of the nipple. This suture is usually permanent and is like a lasso at the base of the nipple. However, this means your nipples would not be flat, and your nipples would be permanently erect. Therefore, please be careful what you wish for. The nipples can be made more projectile, but that means that at all times, they are projectile. That means it may be difficult to conceal nipple erection in certain clothing. This may or may not be an appropriate look for you. However, technically, plastic surgeons can correct flat nipples. Safe operating time is 6 hours. You would not be able to do a tummy tuck, breast lift, breast augmentation, lipo 360 and fat transfers to the buttocks at the same time. I would begin with the breasts using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. At the same time, a tummy tuck can be done. Since you are not planning future pregnancies, I recommend abdominoplasty with mesh reinforcement. The mesh creates a more hourglass figure by compressing the abdominal wall resulting in early satiety and helps maintain a reduced weight. After these have been completed, you can consider liposuction and fat transfers to the buttocks and breasts, if additional volume is desired.

Yes, you are a candidate judging from your photos. A vertical lift is best (not the anchor scar lift). This surgery does not affect the erectility of your nipples. There is no good way to make them more protuberant although grafts have been used and even prosthetics. Be sure your surgeon is experienced, particularly in doing combination procedures. And it is important to know how to be safe doing buttock fat transfer.

From the photos you appear to be a good candidate for a Mommy Makeover. While combining procedures is common and considered safe there is a limit. With all that interests you it may be better off to break these procedures into two stages. This would be for your safety as well as to give your surgeon the opportunity to optimize your results. Your surgeon will advise if it is advisable to have them in one stage once you have been examined and your surgical plan is set.
