Is an augmentation a good solution? (Photo)

Thank you for your question.Large, heavy breasts are likely to drop and often drop quickly.  Adding a breast implant might just make that problem worse.Smaller and perkier lasts longer than bigger breasts.

If it is just the shape, then I would revise the lift.  The added implant weight has the tendency to cause the bottoming out effect sooner.Try to research this to find the surgeon who consistently produces the
results you would expect. Look at reviews on this site and see who gets the surgery right every time. Find
a board certified plastic surgeon who performs hundreds of breast lifts each year. Then look at the plastic surgeon's
website before and after photo galleries to get a sense of who can

The distance from the areola to the infra mammary fold appears to be long. If it's greater than 6 cm a secondary mastopexy may give you better results in placement of the breast implant that in my opinion would make the breasts look excessively large.

I think you should talk to your plastic surgeon about possibly using mesh to support the implant.  Based on what you have said, you are going to likely bottom out because of your skin.  The implants will drop into the lower half of your breast.  I would consider a mesh sling to support the implants and hold them high.  You would need a revision of your lift to maintain the correct nipple position.

In my over the internet opinion I would re lift the boxy result and do fat grating to even out the breasts and give upper pole fullness... 

but I wouldn't consider it a good solution.  You should make a list of what you are trying to achieve and desire and discuss each item with your surgeon.  What technique employed for the lift also makes a difference as to what could be recommended so your records will be needed if you see someone else besides your surgeon.  If you're still uncertain about what was recommended, you can always re-post a question here for commentary. 

 Thank you for the question and pictures. Exactly what operation you will benefit from will depend on your specific/personal goals. Therefore, you will find that online consultants will not be able to provide you with specific enough help to be truly helpful. Best to be seen by board-certified plastic surgeons who can discuss options with you in front of a full-length mirror. Certainly, breast augmentation surgery will increase breast size;  some further “tailoring” of the lower breast skin may help improve shape as well. Again, best to discuss your specific goals and concerns with well experienced board certified plastic surgeons who can demonstrate significant experience helping patients in your situation. In my practice, I find the use of goal pictures as well as computer imaging very helpful during this communication process. I hope this, and the attached link, helps.

To try and simplify, you should consider breast implants as just filler and the result will be making your breast shape larger. If you do not like the shape of your breast then the shape needs to be changed with some sort of breast reshaping or mastopexy. You cannot control or change your breast shape with an implant.
