Thank you for your photographs. I understand that you do not like your chin and neck aesthetics. Certainly those can be addressed with jaw surgery (orthognathic surgery). In my opinion, your temporomandibular joint dysfunction could be improved with jaw surgery, but as you mentioned, it also can be aggravated or worsen by the procedure. If you were purely looking for improvement of your lower third facial aesthetics and neck there are a couple of procedures that can be formed.
One would be simple bone reduction of that area. While the other would be a sliding genioplasty to set your chin back slightly. You may also require management of the skin fat beneath the chin to make this area aesthetically pleasing as well. This may be in the form of liposuction, lower neck muscle tightening and/or skin tightening. My advice would be able to seek consultation from a skilled plastic surgeon or oral & maxillofacial surgeon regarding these issues.

In the photos, you have a deep mentolabial fold which usually indicates a chin that is short in the vertical dimension. In addition, you have a collection of submental fat that is creating a bulge beneath your chin. Depending on your clinical exam, it may be advised to have a vertical lengthening genioplasty and jawline and submental liposuction.

This should be possible. We can adjust the chin bone to bring it back, and the soft tissue under the chin to reduce the sagging you have before.
