Unfortunately, much of the hype, marketing and advertising for many of the so-called "next big thing," relatively expensive bells and whistles devices, whether light or energy-based (and Vivace combines LED light with radiofrequency microneedling), remains largely unproven and backed more by device manufacturer supported marketing than hard science. So, in general, when it comes to all these, a healthy dose of "buyer beware" is the order of the day. Without photos clearly demonstrating the problems, it is hard to say whether you might profit from the use of volumizing fillers for the jowls and skin boosters for the neck. Skin boosters are biostimulatory fillers that promote the synthesis of new, native collagen and elastic fibers, which help to smooth and tighten the skin. In my Israel satellite facility, where a far greater number of regulatory agency approved fillers are available, I typically use Restylane Skin Boosters, Juvederm Volite and Hydromax for this purpose. In my NY office, I generally opt for Restylane Silk. If needed, simple, relatively inexpensive conventional microneedling with or without the addition of a low concentration of TCA may be provide additional smoothing.
