How can I get the round perky look of breast implants without the actual implants? (photos)

The choice of a bread lift may be right for you. The choice of technique can effect the post op shape results. Have a consultation with a Plastic surgeon to discuss your options. The options that will shape the breast the best will involve a peri-areola scar and a vertical scar as well. Good luck.

Hi.  While I'm sorry to disappoint you, I think the honest answer is that it's very hard to get the round "implant" look without using actual breast implants.  The round look may be present for a short time post-op, while tissues are swollen and the skin is relatively tight - but it really doesn't seem to last, even with auto-augmentation or fat-grafting techniques, in my opinion.  I'd challenge any of the surgeons that claim they can do it consistently without implants to show some long-term (1 year out) results.

Thank you for the picture. I think you can receive a round look by just having a breast lift and small reduction. This also equalize the volumes of each breast. Seek in person consultation from plastic surgeons in your area.

in a large drooping breast is certainly a challenge.  And as you can see, there are many options to choose from.  But if you really want perky and full on top, you must consider a 'Rubin' Mastopexy but scars do come with it.  And gravity could tear it apart if the tissue is not anchored properly.  Auto-augmentation mastopexies also do well but do not produce the round look on too.  Both these techniques improve nipple projection.  As for the UBL by Dr. Horndeski, the lack of scars is appealing but is the shape great?  According to one of our journal articles, it doesn't produce the best shape most desire (but certainly less scarring).  None of these procedures will make your breasts touch in the middle as the photo you showed.  Finally, you could consider having a reduction along with augmentation to produce the look you desire if you are willing to give up your native breast tissue and be smaller when the day comes when the implants have to come out.

To perk up a drooping breast, your surgeon will remove
excess skin, reposition the nipple, and redrape and tighten the remaining skin
to support the breast. The areola may also be reduced in size. A mastopexy
(breast lift) raises droopy breasts from one to several inches and excess skin
is removed. I commonly use a minimal incision technique (Le Jour), eliminating
the need for large scars while providing a pleasing breast contour and shape
that is proportionate to the patient’s body. If you're sagging or asymmetrical,
and are looking for perkiness, you'll need a lift. The great thing about these
techniques is that they can now be down with minimal scarring and performed as
an outpatient procedure without the need for general anesthesia.

hithx for the question. based on your pics and the desired shape, a full breast mastopexy would be my preferred technique. I would suggest a small reduction in size. Gravity remains your enemy, and the larger your breasts the more likely the tendency to have them sag slightly. It's unlikely to sag back to the degree you see now, but it's important to be as healthy as possible and maintain good support. If you smoke, you need to stop absolutely. Prices will depend but our fees for a breast lift / reduction runs around $7500. It takes about 1-2 weeks to recover and a month before you can resume light exercise. Preoperative bloodwork / medical clearance will be based on your age and any underlying health conditions. If you have any questions, please let me know. 

If you are satisfied with the size of your breasts then a breast lift with a dermal pedicle autoaugmentation is a real choice for you.  This procedure uses your breast tissue and places it in a pocket where a subgladular breast implant would normally sit.  Your breasts appear to be too long for a vertical type incision so more than likely you would need an inverted T type incision to remove all of the excess skin.
