Which implant size for a small BDW? (Photo)

After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants, and they choose what looks right for them. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. Thanks for your question! The key to a good outcome is picking an implant that fits your body so that there aren't complications created by the size. For more volume, implant replacement down the road is always an option. Every breast is different. Your plastic surgeon is making his recommendations based on your frame, base width, etc. I would trust his judgement, and I'm sure you will get a beautiful outcome. Good luck!

Based on your photos, you appear to be a great candidate for breast augmentation procedure.
Implant size discussion and implant selection is one of the most important steps during your consultation with plastic surgeon. The decision-making process depends on patient’s desires, as well as breast and soft tissue dimensions. Either 285cc or 305cc moderate plus projection breast implant size should be able to give you the look you desire. The difference in volume between these 2 implants is very small/barely noticeable.
I suggest that you seek an in person consultation with a Board-Certified Plastic Surgeon in order to discuss your goals and concerns for the best cosmetic results. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.

After your breast surgery, you would not be happy seeing your nipples on the low side of the breast mound, would you? A very important measurement is the distance from nipple to fold on stretch. This will govern where your nipple sits relative to implant size and regardless of implant profile. Rough estimate of maximum volume for a given nipple to fold distance is: 6.5 cm 225 cc, 7 cc 250 cc, 7.5 cm 275 cc, and 8 cm 300 cc. If you pick a 305 cc implant, but your nipple to fold distance on stretch is 6.5, you will either have low riding nipples, or if your surgeon 'lowers the fold', a high risk for double bubble. Be smart and not greedy; pushing the size in someone like you will lead to a life of complications and reoperations. You can always get larger implants later in a second operation with much more reliability.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

I don’t like the bwd method of selecting sizes. It’s supposed to be precise but in reality it causes many to select the wrong size. Even 300 cc is quite conservative for your build. I wish these doctors would get off this bwd band wagon.

Your surgeon who has examined and measured you will be the best one to guide you to the range you should consider that fits your frame and tissue. If you go larger, you may have more future problems. I think the 280 cc implants will look very nice on you.

Your surgeon has had the benefit of examining you and discussing your surgical goals. If you have selected a board certified plastic surgeon and you feel comfortable and confident in your choice of surgeon I would trust his opinion. It sounds as if you have discussed your desire for the larger implant and he says he will try to accommodate your wishes if it is possible.
