Can Gynecomastia excision be made NOT on the nipple area?

Question

Some years from now I may have to go under the knife for gyno (I've been taking Finasteride for 4 years and recently my right nipple has been tender and slightly puffy - unfortunately, my hair is very important for my career of choice and mental wellbeing, so I won't stop taking it). Is it possible to have a Gynecomastia excision outside of the nipple area? Like performing the cut 2 inches to the side or upwards? I find the areola scars a little obnoxious and would rather have a small scar on my chest

The technique used to eliminate male breasts depends on the amount of fat and breast tissue, as well as the quality of the skin and position of the nipple complex. If the skin tone is good and the nipple position is not excessively hanging, then liposuction alone will yield a good result. However, most gynecomastia patients are best treated with a combination of liposuction along with some amount of tissue/gland removal. In some cases, an incision directly underneath the areola to take out the gland is the best technique. In other cases, a periareolar (around the areola) incision works best.Be sure to consult with board certified plastic surgeons who have extensive experience performing male breast reduction surgeries who can walk you through the surgical technique recommended (and why) and show you several before and after photos of actual patients with a "before" body that looks fairly similar to your situation.The web reference is me in surgery explaining some of the techniques I use.Good luck!

Yes. I routinely performed a metod called the Lateral Pull Through technique. It includes liposucion, ultrasonic liposuction (VASER) and direct excision all through a 1 cm incision located on the side of your chest. I have been using this technique for well over 10 years. It works in about 80% of patiennts. I have had to resort to adding the lower peri areolar scar in only two patients, both of which had a history of taking anabolic steroids and their breast tissue was simply too dense.

Skype or in-office consult would be necessary for evaluation of the extent of your gynecomastia.  Though the conventional teaching has been that liposuction cannot remove breast tissue, I have found that I can remove the breast tissue with liposuction alone. This allows for small segmental reduction of the breast to produce the best contour possible and avoid over resection, cratering, and contour deformities. The perry areolar incision is going to be the most favorable. The further away you make the incision the more difficult The procedure becomes.  This area also heals with the most acceptable scar. You might find someone willing to make an incision elsewhere but I would not recommend it. Best, Mats Hagstrom M.D.
