Lipoma was removed 6 years ago and came back. What procedure will ensure that it is gone for good?

To minimize the risk of recurrence, the most effective approach is a complete surgical excision, ideally performed by a surgeon experienced in deep or complex soft tissue tumors. This means removing not only the visible lipoma but also the surrounding capsule and any extensions where cells might have spread. In cases where lipomas are close to the spine or nerves, surgeons may work alongside neurosurgical or orthopedic teams to ensure maximum removal while protecting vital structures.

Pathology matters. After removal, the tissue should be examined to confirm whether it is a typical lipoma, an intramuscular lipoma, or a rarer variant.

While liposuction or minimally invasive options exist, they are not recommended for complex or recurrent cases because they cannot remove the capsule — and that increases the chance of another recurrence.

Even with the best technique, deep lipomas can occasionally come back, but a carefully planned surgery guided by imaging gives you the strongest chance of long-term resolution.
